Tim Anfilogoff is Head of Community Resilience for Herts Valleys Clinical Commissioning Group (CCG) with a particular focus on supporting family carers, social prescribing and promoting community led health.

Tim designed the award-winning Community Navigators Service in West Herts which helps people to take control over their health and wellbeing through social prescriptions (finding non-clinical solutions to support them in their communities which have preventive clinical impact). This service also reduces the demand on primary care services from those who do not know where else to go.

Tim also produced the Herts Valleys CCG’s first ever carers’ strategy delivering 69 carers’ champion roles in primary care across West Herts and is now the Carers’ Lead for the Herts and West Essex Sustainable Transformation Partnership.

Previously, as Head of Community Wellbeing for Herts County Council, Tim commissioned £15m worth of voluntary sector and prevention services, during which time he developed the spec for HertsHelp an expert triaging service for voluntary and community assets, which now underpins social prescribing in the County and making onward referrals to 140 different community organisations per month.

Tim managed Hertfordshire multi-agency carers’ strategy for many years, which achieved Beacon status for Supporting Carers in 2005 and partnered the European Action for Carers in Employment (ACE) 2005-7. He managed the National Carers’ Strategy and Carers Grant for the English Department of Health from 1999-2001. He also wrote practice guidance on the Carers and Disabled Children Act (2000).

Tim co-authored Making Sense of Social Prescribing (2017) and co-chairs the East of England regional network as one of NHS England’s regional social prescribing facilitators.

Tim speaks nationally (and internationally) on carers and social prescribing and is currently mentoring six community health centres in Ontario on the development of social prescribing.

Tim was appointed a trustee of Carers UK in October 2017.
